Each referral is assessed against the property, location, peer group, staffing requirement, safeguarding information and agreed support plan. Unsuitable referrals are declined in writing.

Commissioners can request our safeguarding, training and reporting evidence as part of due diligence.
Care leaver accommodation, within our 16+ service
Care leaver placements sit within our 16+ service. The support plan can cover daily living, education, employment, health, relationships, housing applications and move-on arrangements.
Property and location
The property, the neighbourhood and who else lives there are checked against the placement plan.
Named keyworkers
Each young person has a named key worker and Service Manager, with responsibilities recorded in the placement plan.
Education & employability
Support with college, apprenticeships and actually getting there, where the plan includes it.
Identity & relationships
Family contact, culture and faith, and any professional support already in place, are covered in reviews.
Pathway planning
Pathway plan drafted within 10 days of intake, reviewed monthly with the young person and the leaving care team.
Transition planning
Future accommodation, services, applications and changes to support are planned with the young person and allocated worker.
